Who is CMK?
CMK is a division of PerSys Medical (www.ps-med.com). We are headquartered in Houston, Texas. When you call our facility (888-737-7978) we will answer as PerSys Medical.
Why is your staff so nice and friendly?

What is the benefit of creating an account?
Creating an account allows you to shop repeatedly without manually re-entering billing and shipping information. Creating an account also automatically qualifies you to receive special offers via e-mail that are available to account holders only.
Can I make a purchase without creating an account?
Yes you can, however you will not have access to special offers made available to account holders only.
Can I place an order if I am located outside the United States? (link to Shipping information)
No. Our products are intended for sale and use within the United States only.

Can my order be shipped to an APO or an FPO address? (link to Shipping information)

Can I also make purchases and ask questions by phone?
Yes. To purchase by phone you can call our customer service department at 888-737-7978 Mon-Fri
from 8am-6pm CST.
If a product is marked “Out of Stock” can I still place an order for that product?
Yes, orders can still be placed for products marked “Out of Stock” by calling our customer service department at 1-888-737-7978.
What forms of payment to you accept?
We accept CMK gift cards, Visa, MasterCard, American Express and Discover. We also accept government purchase cards (major credit cards). We do not accept checks or money orders.
Why must I provide my billing address, CID# and zip code when ordering by credit card?
It is imerative that we protect our customers, and entering complete billing and credit card information allows our credit card vendor’s system to fully validate the cardholders and the card for better security. Credit card charges cannot be processed unless all information is completed.
How is shipping and handling calculated? (link to Shipping information)
FedEx SmartPost® and FedEx Ground® calculate shipping and handling by the total weight of the items being purchased, the ship to zip code and the service type requested.
Why do I need to prove that I am qualified to purchase certain items?
Some of our medical items are only to be used by certified/trained professionals. Before you are able to purchase these items an agreement check box will appear at checkout requiring you to agree to our terms and conditions.
How do I know that you received my order? Will I receive a confirmation?
Yes, an e-mail confirmation will be sent once your order has been placed.
When will my order ship? (link to Shipping information)
Orders are normally shipped within 3-7 business days if all items are in stock.
Can I track my order?
Although online tracking is not available through the CMK website, all orders can be tracked by contacting our customer service department.
When I order a bag and medical items, does CMK pack the items into my bag?
No. If you order a bag and items you will receive all items separately. This way you can “build your own kit” and place the items in the areas they will best serve you.
Can I return items? (link to return policy)
Returns are accepted for items that are unopened, unused and still in original packaging with tags attached within 30 days from date of purchase.
Do I pay sales tax?
Sales tax is added to all orders that deliver within the state of Texas.
Do you have a retail store?
No. CMK is a division of PerSys Medical headquartered in Houston, Texas. We do not have a retail store.
I have questions about charges on my credit card. Who do I contact?

Can I add to an order once it’s been submitted?
Yes, but you must have your order confirmation number. If you submit an order online and then need to add an item, please call customer service at 888-737-7978 to add items.
Can I have my order shipped to an address that is different than my billing address?
Yes, if the ship to address is located within the United States.
How do I calculate shipping before checking out? (link to Shipping information)
Shipping and handling can be calculated by using the shipping calculator located in the shopping cart.
If I have suggestions for new products or for the website, does CMK want to hear them?
